Reaching underserved communities

Designing targeted outreach for people experiencing health inequalities and other barriers to accessing information, services and opportunities.

For a Sport England-funded place-based physical activity programme in Brent, we developed the research and outreach strategy that gathered insight from 1,067 residents, working through trusted faith, healthcare and community networks to reach diverse communities and understand what would help more people become active.

Creating and managing online communities where people can access practical information, ask questions and learn from others with shared experiences.

For a North West London diabetes programme, we developed an active peer-support community alongside accessible health content, expert-led Lives and Q&As, helping people engage with diabetes information in a more approachable and supportive environment.

Supporting participation and social connection

Promoting physical activity and wellbeing in ways that recognise the wider reasons people engage and the barriers that can prevent participation.

Our work with a Harrow community walking programme included supporting older adults and people with long-term health conditions, using accessible communications and community engagement to build confidence, encourage participation and promote peer support and social connection that help people stay active long term.

Our work in action

MyHealth London

The brief
Increase awareness and uptake of an NHS-supported digital cardiovascular health programme across North West London, with a particular focus on reaching communities experiencing greater health inequalities.

What we did
Developed and delivered the multi-channel marketing and engagement strategy, combining targeted social media and paid campaigns with accessible health content, video, webinars, community outreach and printed materials. We created clear routes from initial awareness through to registration and programme participation, working within NHS clinical review and governance requirements.

The outcome
We built the programme’s social presence from zero, generating 18,400+ organic content views, 490+ interactions and 522+ profile visits, with Facebook reaching around 6,500 people without paid advertising. Across the wider programme, total registrations increased 22% to 25,105, while cardiovascular registrations rose from 3,158 to 12,624, with our activity supporting sustained awareness, engagement and visibility.

Brent Active Together

The brief
To gather insight from diverse communities experiencing health inequalities, deprivation and multiple barriers to physical activity, helping to shape a new Sport England-funded place-based programme for Brent. We were then tasked with developing the communications and engagement approach needed to reach and involve local communities.

What we did
We designed the community research and outreach strategy, gathering insight from 1,067 residents through trusted local networks including faith, healthcare and community settings. We then developed the brand and communications strategy, website and digital channels, accessible campaigns, partner resources, promotional materials and event support, alongside training teams to support ongoing delivery.

The outcome
We established the communications and engagement infrastructure needed to support long-term place-based delivery and reach communities facing barriers to being active. The work was subsequently showcased as an example of good practice at a national Sport England conference, and following the successful Development Award phase, Brent became the first Place Partnership in the country to progress to the full Sport England award.

Friends of Know Diabetes

The brief
To create an accessible and trusted way for people living with or at risk of diabetes to access practical health information, connect with others and engage with healthcare professionals outside traditional clinical settings.

What we did
We developed and managed the online community and social channels, creating clear, bite-sized health content alongside peer-led and lived-experience communications. We delivered regular Lives and Q&As with healthcare professionals and high-profile guests, supported community conversations and developed engaging content within NHS governance and approval requirements.

The outcome
We grew the Facebook community to 1,300+ members, with around 65% actively engaging, alongside an Instagram audience of 2,400+ followers. This created an established peer-support network where people could access practical diabetes information, learn from lived experience and engage directly with healthcare experts in an accessible, supportive environment.
